§ 5.20.040. Physical examination of bakery employees.

  • Prior to the issuance of a license for any bakery, the applicant shall present to the village collector a list of his or her employees and a certificate of a duly licensed physician and surgeon of the village dated within thirty (30) days of such application, certifying that the employees of the bakery are free from venereal or contagious disease. Applicant must also make affidavit on a form required by the collector that the list of names of employees submitted with such application contains a full and complete list of all employees of such bakery. In case of a new employee during the fiscal year, a certificate showing the condition of health of the new employee shall be presented to the village collector within three days after the employee enters upon such employment. At the time of the renewal of the license such medical certificates shall again be furnished.
